Apply now to see what it means to us and what it's like to work somewhere your voice is heard, your wellness is a priority, and your success matters.JOB TITLE:Community Support Specialist IIEMPLOYMENT TYPE:Full-time, 40 hours/wkSCHEDULED HOURS:Monday-Friday 8:30am-5:00pmPROGRAM/LOCATION: Young Adult Community Treatment Team, Enfield, CTPC#:1029ABOUT THE PROGRAM:Young Adult Community Treatment TeamThe YACTT is an intensive community support program for young adults at risk of serious mental health, addiction, and/or co-occurring disorders.The program consists of a multidisciplinary team of case managers, therapists, an employment/educational specialist, and a program director to serve individuals living in the community with additional nursing and psychiatric support. Through the efforts from the team, the program teaches young adults life skills, recovery skills, help with employment and education, and provide support to recover from their mental illness and addiction.As an outreach team, we are able to break down barriers that can interfere with progress by meeting clients in the comfort of their environment.
