Summary:

Provide direct care and supervision to all program participants; focusing on health and safety, skill acquisition, social skills, daily living skills, self-advocacy skills, and community participation, in accord with individual choice and human rights. Work in a Direct Care capacity to support community integration, staff vacancies or absences. Collaboratively develop, implement, and maintain a weekly schedule of programming and activities. Monitor program attendance and record in electronic database. Complete HCSIS ISP Documentation as assigned and supervise HCSIS ISP documentation of Direct Care staff to assure quality and that timelines are met. Ensure completion of all Progress Notes, Support Strategies and other required documentation utilized within the program in a timely manner, ensuring all timelines are met. Act as a liaison between The Price

Center/families/DDS/other

external agencies in collaboration with the Director. Model a professional appearance and demeanor while at the program and while in the community with individuals. Assist the Director in managing and supervising the day-to-day operations of the Community-Based Day Services Program. Assist the Director with formulating, implementing, and monitoring program goals and objectives Assist with updating and maintaining Electronic Healthcare Record System. Other duties as assigned

Requirements:

High School Diploma.

Two (2) years’ experience in a community-based day program.

Experience working with persons with developmental disabilities.

Strong communication skills. Demonstrated leadership skills. Ability to interact with

individuals/staff/families

in a professional manner.

Possession of a valid Driver’s license, a clean driving record and a willingness to use personal or agency vehicle in the course of employment.

Ability to lift and support body weight of any program individual with assistance of another employee. Current MAP, CPR, and First Aid Certifications, Proficient with Microsoft Office

Preferred:

Bachelor’s Degree and/or two (2) years’ experience working with persons with developmental disabilities in a supervisory role.
