Responsibilities:

What You Will Do:

  • Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes conduct patient home visits
  • Provide personal care to our patients, including dressing, mobility, administering medication, personal hygiene, eating and transporting to doctors appointments
  • Communicate with family regarding patients condition
  • Document and timely report any changes in the patients health status
  • Timely complete records in accordance with Company policy


Qualifications:

Qualifications:

  • High school diploma
  • Valid drivers license
  • CNA state certification (required)
  • Prior experience as a CNA (required)
