The position is responsible for therapeutic assessment, interventions, and plans for individual patients as well as patient populations. The Pharmacist is a valued member of the Interdisciplinary Care Team and assumes responsibility for medication-related outcomes. The pharmacist aligns with the pharmacy enterprise's vision of improving outcomes and bringing value to our patients. The Infusion Therapy Pharmacist will review oncology and other infusion therapy treatment regimens for patients for three outpatient clinic locations and be involved in the supervision of compounding and delivery of these medications. The infusion pharmacy services the greater Wenatchee area as well as satellite clinics in two rural cities, Omak and Moses Lake. The Oncology team is composed of physicians, nurse practitioners, social work, palliative care, hospice services, nurse navigators, research, financial services and pharmacy. The Wenatchee pharmacy will have approximately 70 patients per day, with Moses Lake and Omak averaging 30 patients per day.Position Reports To: Pharmacy Manager Essential FunctionsEnsures safe, effective, and cost-effective use of drugs on behalf of the patient through the application of specialized knowledge, skills and professional judgment, while considering the patient's individual condition.Adheres to all federal and state regulations pertaining to the practice of pharmacy.Provides clinical and financial evaluation and documentation of medication selection, administration and delivery.During outpatient oncology shifts, the clinical pharmacist is responsible for evaluating patient treatment regimens, verifying electronic orders for oncology and other infusions, supervision of sterile compounding, assuring all medications are dispensed safely and in accordance with Confluence Health policies and procedures, providing drug information to oncologists and nursing staff.Maintains a charting system that meets all clinical and fiscal requirements.Provides audience appropriate drug information to providers, nurses, and patients.Participates and contributes to department and organizational performance initiatives.Provides direct supervision of pharmacy technicians and assistants to appropriately maintain the provision of safe, timely and effective pharmaceuticals.Commits to instructing, modeling and coaching pharmacy staff, students and residents.Reports adverse drug events and mitigates future patient safety risks through participation in medication quality activities.Appropriately identifies and communicates opportunities of improvement to the medication use system and assists in solution implementation.Ensures participation and compliance with hospital and departmental policies, procedures, and practice guidelines.Understands handling of Hazardous Drugs.Basic knowledge of USP 797 and USP 800.Required to maintain active Basic Life Support (CPR) certification.Other duties as assigned.Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times. QualificationsRequired:Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy degree from an accredited school of pharmacy.PGY1 residency training and/or equivalent of three years as a licensed pharmacist in the anticipated practice area.Licensed to practice pharmacy in the State of Washington prior to start date.BLS within first year of employment, and must be maintained.Must possess basic computer skills.Desired:1-2 years' experience in infusion.Bilingual with the ability to pass a fluency evaluation with a CH chosen vendor.Strong communication and leadership skills. Requires working under stressful conditions in which problem solving skills are essential. Appropriate supervision over pharmacy technicians and assistants is required. Day shift with variable times including 8 and 10 -hour shifts. The position will require a rotation of telepharamcy supervision for the Omak and Moses Lake pharmacy locations, all in Wenatchee. The Infusion team also provides coverage for Home Infusion and will have after hours on-call coverage.
