Job Summary The Case Manager is responsible for providing a range of services for clients with either Psychiatric and/or Substance Abuse issues. Also provides advocacy, outreach, and service linkage for clients and initiates access to entitlement services.Qualifications Experience working with a behavioral health and/or chemical dependency population. Bachelor's degree in Human Services or related field, Social Work preferred. Valid driver’s license may be required based on program. Knowledge of community resources preferred.
