About Us JGS Lifecare, located in the quaint town of Longmeadow, MA, has been the proud caretaker of the elderly within the local community for over 100 years Founded on the Jewish principle of respecting age as a blessing, we have dedicated ourselves to delivering the best in elder care services to people of every faith and every background. Our spacious campus has a full array of services including Skilled Nursing, Senior Living, and Community programs that consist of traditional long-term care, short-term rehabilitation, memory care, assisted living, hospice, and day health support services. Our employees are an outstanding team of professionals who go above and beyond each day with their compassionate care to our residents and the individuals we serve. JGS is looking for a Speech-Language Pathologist to join its team. The SLP evaluates, treats, and rehabilitates clients by providing them with necessary therapeutic intervention. The SLP maintains pertinent documentation, carries out appropriate treatment plans, and contributes to discharge planning. We Are Looking for Someone Who Can: Complete screens (as scheduled) Complete evaluation and accompanying documentation at the time of the evaluation Obtain and verify physician orders prior to initiation of evaluation Identify and secure payer source prior to initiation of evaluation Identify objective findings and problems Establish appropriate short/long term goals Document progress in attainment of goals on a timely basis Utilize current body of knowledge, skills, and techniques Interact with clients/families in an appropriate, safe, and professional manner Design adaptive equipment and orthotics Design and/or implement home programs as appropriate Communicate appropriate referrals to team members and/or supervisors Assist in the development and implementation of new programs/techniques to enhance therapy services (as assigned) Balance and maintain an appropriate caseload to meet company and agency productivity standards Initiate and follow through with restorative programs as appropriate Attend patient care meetings (as assigned) and actively participate Follow through with discharge planning, including: equipment ordering, family teaching, home safety recommendations, and discharge documentation (as appropriate) Maintain QI documents accurately (as assigned) Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. We Are Looking for Someone Who Is: Master's degree (M.S.) from a four-year college or university; related experience and/or training; or equivalent combination of education and experience.
