Social Services Supervisor

Foothill Heights Care Center -
It is the primary function of the Care Giver to perform restorative nursing procedures that maximize the residents’ existing abilities, emphasize independence instead of dependence, and minimize the negative effects of disability with an attitude of realistic optimism under the supervision of a restorative nurse.
· Immediate supervisor: Day supervisor/ Administrator
Delegation of Authority: Administrative authority, responsibility and accountability necessary for carrying out assigned duties.
· Basic Requirements:
Knowledge, Education and training:
1. High School diploma desirable.
2. At least 18 years of age.
3. Completion of the care giver program/ certificate.
4. Possess First Aid certification
5. Must be able to read, write, speak and understand the English language.
6. Must possess basic knowledge of nursing practice and procedures, as well as laws, regulation and guidelines that pertain to assisted living.
7. Knowledge of resident rights including but not limited to Patient Self Determination Act. Right to execute an Advanced Directives, right to treatment/refuse treatment, equal access to care, right to privacy & confidentiality of information etc.
8. Knowledge to survey process.
9. Require Health Screening, TB clearance and criminal record clearance.

1. Must be able to understand and carry out written and verbal instructions.
2. Must be able to communicate with other coworkers.
3. Must possess the ability to make independent decisions when circumstances warrant such actions.
4. Must possess the ability to deal tactfully with personnel, residents, family members, visitors and the general public.
5. Must be able to present self well in various situations manifested by pleasant/ good grooming.
6. Must possess the ability to assist residents, visitors or the general public in matters that concern general nursing services.
Attitude:
1. Must possess a positive attitude toward work, peers, staff, residents, families, visitors or physician, discharge planners or general public.
2. Must possess a positive attitude towards problem identification, and problem resolution.
3. Must be cooperative with initiative and team spirit
4. Must possess the ability to work harmoniously with other personnel
General Duties/ Responsibilities:
1. Follow established performance standard and perform duties according to the expectation.
2. Report all accidents and incidents observed in the shift as they occur.
3. Provide restorative nursing care as assigned, completing assignments accurately and timely.
4. Identifies safety hazards and emergency situations and initials corrective action immediately.
5. Provide restorative nursing care to residents without violating residents’ right.
6. Participates in facility education programs as assigned.
7. Transport residents to treatment area, as needed.
8. Demonstrates an attitude of cooperation and enthusiasm, during instruction and in resident care activities.
9. perform all assigned tasks in accordance with established policies & procedures, and as instructed by supervisor/ administrator.
10. Follow work assignments and / or work schedules in completing and performing assigned tasks.
11. Report all grievances made by the resident to the supervisor/ administrator
12. Notify the supervisor if you will be late or absent.
13. Report occupational exposures to blood, body fluid, infectious materials and hazardous chemicals to supervisor/ administrator.
14. Report and work-related incidents to the supervisor/ administrator.
15. Create an atmosphere of warmth, personal interest, and positive emphasis, as well as calm environment through the facility.
16. Participate in facility’s surveys made by authorized government agencies as requested by the Administrator.
17. Observe, maintain and enhance resident rights.
18. Report all allegations of resident abuse and or/ misappropriation of resident property.
19. Follow establishment procedures in the nurse and disposal of personal protective equipment. Perform accurate and timely charting of the resident accordingly.
20. Sign and date all entries made in the resident’s medical record.
21. Maintain an excellent working relationship with other staff and coordinate the nursing services to assure that daily needs of residents can be provided without interruption.
22. Others as may be directed by the supervisor/ Administrator.
Responsibility:
1. Provide meals for residents daily and according to the menu schedule
2. Clean resident’s room, do laundry weekly
3. Provide social activities for resident such as game, reading books, outdoor activity, watching movies, shopping.
