JOB PURPOSE:
The candidate will lead and oversee the patient assessment function in the intake process, ensuring the delivery of high-quality and comprehensive assessment services. This leadership role involves strategic planning, program development, and collaboration with multidisciplinary teams to enhance patient care outcomes. The Director of UAS directs the clinical activities associated with the entire Intake process and assumes responsibility for final determination of the clinical suitability of prospects for enrollment into the organizations clinical programs.

JOB RESPONSIBILITIES:

Develop and implement the strategic vision for patient assessment services, aligning with organizational goals and objectives.
Provide strong leadership to a team of assessment professionals, fostering a collaborative and patient-centered environment.
Ensures that documentation by Intake Nurse Managers is timely, accurate, appropriate, and meets timeframes as determined both by the department and governmental agencies.
In conjunction with the Marketing and Business Development team, assists in identifying referral trends that will lead to the conversion of appropriate patients into the program and improve the overall quality of enrollments.
Evaluate and enhance existing assessment protocols and procedures to optimize patient outcomes.
Design, implement, and manage patient assessment programs that meet regulatory standards and industry best practices.
Collaborate with medical and clinical teams to integrate assessment services seamlessly into patient care pathways.
In conjunction with the Nursing Department, oversee the authorization of personal care hours to ensure appropriateness and consistency with the identified needs of the participant and Medicaid regulations.
Establish and enforce quality assurance standards for patient assessments.
Monitor and evaluate the effectiveness of assessment protocols, making adjustments as needed.
Ensure compliance with relevant regulations and accreditation requirements.
Collaborate with physicians, nurses, and other healthcare professionals to enhance the integration of assessment data into patient care plans.
Facilitate communication and coordination among various departments to streamline assessment processes.
Oversee the collection and analysis of patient assessment data.
Generate reports to communicate key findings and trends to leadership.
Utilize data to drive continuous improvement initiatives.

QUALIFICATIONS:

Education:

BSN - Registered Nurse with current license in New York.

Experience:

Minimum five (5) years of experience in Intake, Enrollment, quality assurance or discharge planning (preferably within a managed care, home care, or community health organization)
A minimum of two (2) years of experience in MLTC/Managed Care with preparation of the Uniform Assessment System of New York (UAS-NY) is required.
