Job Summary: The Patient Navigator-RN Bariatric Services serves in a Magnet recognized tertiary academic medical center as a key member of the multidisciplinary healthcare team providing comprehensive care for patients in the bariatric medicine and surgery program. Demonstrates a comprehensive body of knowledge related to the specialty practice of bariatric medicine and surgery. Ensures patients, families, and their care givers receive needed education and support while they are guided efficiently and effectively through the continuum of care. Readily accepts and evaluates referrals for recommended bariatric surgery based on established criteria. Performs the nursing process (assessment, care planning, intervention, evaluation) in accordance with LLUMC's established Professional Practice Model (Duffy's Quality Caring Theory) to achieve proficient individualized physical, psychosocial and spiritual wellness and provide compassionate bereavement support as appropriate. Intervenes as a patient advocate as necessary. Works in tandem with the program leadership and the Medical Director and surgeons to acquire and maintain program accreditation through the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP).Maintains databases as required and enters all data accurately and timely Collaborates with the medical director and surgeons to develop and implement clinical practice policies, procedures and pathways that are parallel with best practices for all aspects of care related bariatric medicine and surgery. Engages in opportunities to directly or indirectly influence decision-making for clinical practice by supporting and participating in committees, task forces and staff meetings. Participates in quality improvement initiatives. Excels in implementing new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Participates in Institutional Review Board (IRB) approved research as requested. Exhibits refined interpersonal skills that portray professionalism and maximize excellence in customer service and safe patient care. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed. Education and Experience: Bachelor's of Science degree in Nursing required. Master's degree in Nursing preferred. Minimum three years of medical/surgical inpatient nursing experience required. Minimum one year of bariatric nursing experience or prior experience as service line Navigator preferred. Knowledge and Skills: Demonstrates comprehensive knowledge related to the specialty practice of bariatric medicine and surgery. Ability to apply theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in area of specialty and population served. Thorough knowledge of the criteria for Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP). Basic knowledge of healthcare regulations and accreditation required. Good understanding of data base management, able to enter data and extrapolate date. Able to read, write legibly and speak in English (and Spanish preferred) with professional quality; use computer, printer and software programs necessary to the position, e.g., Microsoft Office, Outlook, electronic event reporting program, established organization electronic medical record; operate and troubleshoot equipment utilized for position; perform basic patient care activities using proper body mechanics and safety precautions; comprehend and follow-through with instructions; relate and communicate positively, effectively and professionally with others; be assertive and consistent in following and enforcing policies; collaborate; accept direction; think critically and use good judgment; problem solve and collaborate with licensed care providers as necessary; perform duties with minimal supervision; perform basic math functions; organize and prioritize workload; problem solve; recall information with accuracy; pay close attention to detail; distinguish colors as necessary for position; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place and patient care; discern temperature variances through touch; see adequately to read computer screens, medical records and written documents necessary to position, exhibit professional behavior at all times. Licensures and Certifications: Active California Registered Nurse (RN) required. Basic Life Support (BLS) certification issued by the American Heart Association required.
