JOB SUMMARY

The Patient Navigator works in collaboration with a multidisciplinary team, including Case Management, Quality, Physicians and APPs, and clinical managers, functioning as a liaison between patients and healthcare professionals. The Patient Navigator is involved with goals of care, especially as it relates to chronic conditions, critical illnesses, challenging placements, and helping to provide education, resources, advocacy, and mitigation for complex next levels of care.

Job Specific:

  1. Assist and facilitate the transition of care, helping to identify and mitigate barriers to bridge patient and family expectation with treatment goals. (EF)
  2. Clarify options to patients on their choices as it pertains to post-acute care. (EF)
  3. Solve conflicts and crisis situations with health care professionals and family. (EF)
  4. Provide education and resources to staff, patients, and family to help them gain an understanding of complex next levels of care. (EF)
  5. Ensure patient self-management support is available when appropriate based on language, cognitive abilities, literacy levels, learning style, cultural norms, patient preference, readiness for change, and resources available. (EF)
  6. Serve as a supportive resource for staff, patients, and family, assisting clinicians in explanation of care planning. (EF)
  7. Maintain professional growth and development through education of current, evidence-based healthcare practices and trends.
  8. Complies with applicable regulatory agency guidelines, and federal, state, and local laws, rules, and regulations. (EF)
  9. Participate in hospital committees, hospital-sponsored programs and activities, and community activities, as requested.

Hospital:
1. Adheres to organization-wide and other applicable policies and procedures.
2. Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations.
3. Performs within the prescribed limits of the hospital's Ethics, Compliance and Confidentiality Program guidelines.
4. Performs within his/her professional Scope of Practice at all times.

Minimum Requirements

EDUCATIONAL REQUIREMENTS

Required: Bachelor's Degree

EXPERIENCE

Required: Minimum of 5 years' healthcare experience

Preferred: Experience in developing care plans for a diverse, complex patient population

SPECIAL SKILLS AND ABILITIES

Required:
  • Strong communication skills and ability to round on patients/families.
  • Listening skills to understand what the patients' needs are and whether they have any problems or concerns.
  • Problem-solving skills to address possible patient concerns with healthcare professionals.
  • Compassionate attitude to give emotional support and help the patients' quality of life.
  • Expertise in healthcare processes, critical thinking, and problem-solving.
  • Strong organization skills to keep track of complex issues and solutions
LICENSURE / CERTIFICATION REQUIREMENTS

Preferred: BCPA (Board Certified Patient Advocate)
