Key Responsibilities and Duties:
In this role, you'll be at the forefront of guiding our clients towards their best state of wellness, leading a team of caregivers with precision and compassion. Here's what you'll be responsible for:
- Holistic Assessment: Oversee the comprehensive assessment and evaluation of clients' needs, working closely with families to achieve and maintain optimal wellness.
- Resource Facilitation: Ensure clients receive the right resources and support for their care and wellbeing by facilitating interactions with various providers.
- Strategic Decision-Making: Take charge of decisions related to resource allocation, approach, and tactical operations within your department, driving forward key projects and initiatives.
- Collaboration Champion: Engage in cross-departmental collaboration, participating in meetings with fellow team members to align our efforts with company objectives.
- Regional Connectivity: Maintain regular contact with other care managers across the region, fostering a network of knowledge and support.
- Team Stewardship: Lead the recruitment, onboarding, and, when necessary, the termination of caregivers within your scope, ensuring our team is built on excellence.
- Versatility: Be prepared to take on other related duties as required, contributing to the dynamic success of our team.
To excel in this role, you should bring the following qualifications:
- Relevant Experience: Ideally, you'll have 2+ years of experience in home care, showcasing your dedication to exceptional client service.
- Educational Foundation: A high school diploma is required, while a bachelor's degree or equivalent work experience is preferred.
- Administrative Acumen: Demonstrate office and administrative support skills to manage the intricacies of your role.
- Effective Communication: Your excellent written and verbal communication skills are crucial in guiding your team and interacting with clients and families.
- Mobility: A reliable vehicle is essential for travel within your designated area.
