Hospice Rn -- Weekends (Case Manager) This nurse provides end-of-life care to hospice patients in their homes on weekends, helping manage symptoms, supporting families, and ensuring patients remain comfortable and dignified through their final stages of life. Start your shift by reviewing your assigned hospice patients and planning your travel route. Drive to each home to complete assessments and provide compassionate, hands-on nursing care. Monitor symptoms such as pain, breathing difficulty, anxiety, or discomfort and adjust interventions as needed. Teach family members what to expect as their loved one's condition changes. Provide emotional support, reassurance, and education to caregivers and family members. Collaborate with physicians, hospice aides, social workers, and chaplains to coordinate all aspects of care. Update care plans when changes in the patient's condition occur. Document each visit carefully and completely in the electronic medical record. Serve as a resource for aides and newer team members who may need guidance.
