Job Title: Clinical Pharmacist Memorial Healthcare System is seeking a skilled Clinical Pharmacist to join our team in Hallandale, Florida. Job Summary We are looking for a highly motivated and experienced Clinical Pharmacist to work in a dynamic environment. The ideal candidate will have a strong background in pharmacy practice, excellent communication skills, and the ability to work effectively in a team. Responsibilities Provide clinical pharmacy services to patients, including medication therapy management and disease state management. Collaborate with healthcare teams to develop and implement patient care plans. Conduct medication use evaluations and provide recommendations for improvement. Participate in quality improvement initiatives and contribute to the development of policies and procedures. Stay up-to-date with the latest advancements in pharmacy practice and apply this knowledge to improve patient care. Requirements Doctor of Pharmacy degree from an accredited institution. Active licensure as a pharmacist in the state of Florida. Minimum of 1 year of experience in a clinical pharmacy setting. Excellent communication and interpersonal skills. Ability to work effectively in a team environment.
