Job Summary:
The Care Manager provides comprehensive, integrated care management and coordination for health and behavioral health care to individuals with co-occurring chronic conditions.
To be eligible for this position, candidates must have their own reliable vehicle.
Must Have Qualifications & Skills:
- Associates Degree in Health / Human Services
- Valid NYS Drivers’ License
- One year experience of working with recovery supports, community resources, housing, employment, & other professional and nonprofessional services
- Experience in a behavioral health care setting
- Excellent written & verbal communication skills
- Ability to multi-task
- Demonstrates composure in challenging situations
Nice to Have Qualifications :
- Bachelors Degree or higher in Health / Human Services
- 2+ years experience working in Behavioral Health
- Experience in Crisis Management
