Job Summary: Primary responsibilities involve assembling, monitoring, maintaining, retrieving, and filing patient information in the electronic health record in accordance with established Health Information Management (HIM) policies and procedures. Proficient in Release of information, HIPAA compliant, Warrants/Subpoena's, Guardianship, and Designation of Personal Representatives including Electronic submission of birth certification to State of New Hampshire, Vital Records. Maintenance of Master Patient Index.
Essential Qualifications:
High school diploma or GED required. Associates degree preferred. Computer experience is required. A good command of grammar and spelling is necessary. Must possess strong verbal and written communications skills. Medical terminology is preferred. Able to work efficiently in a distracting environment, adapting to frequent interruptions, while maintaining organization and efficiency with essential job functions.
Essential Function:
- Maintains the master patient index (MPI) integrity within the current and old systems, manages multiple reports and takes the necessary actions to correct any discrepancies.
- Notifies HIM Manager, Privacy Officer and Risk Management of any incidents in documentation that may indicate a potential liability issue.
- Complies with NH and federal laws regarding release of information and privacy and hospital-specific policies including STAT requests, reporting the Accounting of Disclosure.
- Gathers, organizes, reconciles, manages, scans and upload patient records to Electronic Medical Record in a timely manner for patient care, quality and coding/billing.
- Completion/quantitative analysis/deficiency process for all services provided within the facility, reports deficiency to appropriate staff and monitor for completion.
- Responds to Patient Portal queries, such as enrollment requests, or assistance with password resets.
- Continuity of Care Documents are electronically sent to appropriate institutions at patient discharge.
- Responsible for the accurate and timely submission of electronic birth certifications using the State of New Hampshire Vital Records prescribed software.
- AVH staff push x-ray images to DHMC electronically and burns images to CD as requested.
- Responsible for retrieving patient records as requested by other staff and/or departments for review. Retrieves and distributes patient records for Professional Practices.
- Receive new patient packets, log, notify appropriate departments, scan documents per policy. Monitors fax inbox, including prescriptions.
- A working knowledge to deal with HIPAA compliant, Warrants/Subpoena's, Guardianship, and Designation of Personal Representative requests.
