Job Objective:
A brief overview of the position.
Offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers utilizing specific clinical knowledge. Using the nursing process provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the care continuum. Works with patients referred for assistance in accessing and utilizing health care resources due to their complex chronic illness.

Qualifications
Education
Required: Bachelor's degree in Nursing
Preferred: Master's degree in Nursing
Licensure/Certification
Required: California RN licensure, American Heart Association BLS
Preferred: Certified in specialty practice area
Experience
Required: Five (5) years of registered nursing experience
Preferred: Inpatient, outpatient, chronic disease management, case management, utilization management, and adult acute care experience
Essential Responsibilities
Demonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violations.
Collects comprehensive data pertinent to the patient's health or situation.
Analyzes the assessment data to determine the diagnoses or issues.
Identifies expected outcomes for a plan individualized to the patient or the situation.
Develops and implements a plan that prescribes strategies and alternatives to attain expected outcomes.
Evaluates progress toward attainment of outcomes.
Enhances the quality and effectiveness of nursing practice systematically. Participates in Patient-Centered team meetings and quality improvement initiatives through the Nursing Practice Council.
Attains knowledge and competency that reflects current nursing practice. Facilitates health and disease education for patients, including leading group office visits. Supports patient self-management of disease and behavior modification interventions.
Evaluates personal nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules and regulations. Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans.
Interacts with and contributes to the professional development of peers and colleagues. Participates on a team for data collection, health care outcome reporting, clinical audits, and programmatic evaluation.
Collaborates with patients, families, and others in the conduct of nursing practice.
Integrates ethical provisions in all areas of practice. Utilizes the ethical decision making model to drive issues and advocate for patient wishes.
Integrates research findings into practice. Monitor the progress of patient through the service plan and re-evaluate the effectiveness and appropriateness of the plan based on the patient's condition and needs.
Considers factors related to safety, effectiveness, cost and impact on practice in the planning and delivery of nursing services. Provide patients with linkages to services to promote a pathway to care and positive outcomes.
Provides leadership in the professional practice setting and the profession.
Practices within the scopes and standards of practice set forth by the American Nurses Association and professional organizations specific to specialty.
Rounds on all oncology treatment inpatients, anticipating discharge needs from admission date and communicating needs to Physician and Case Management staff.
Ensures all follow-up calls for discharged patients are done daily to prevent any unnecessary hospital readmissions per OP-35, by communicating pertinent issues to the oncologists and assisting in the coordination of care for our discharged patients.
Monitors and follows oncology patients who are insured and are placed within their respective insurance allowances (Riverside County Hospital, Borrego Springs, VA, etc.). Builds relationships with these agencies to close the loop within the pipeline of insurances that provide benefits so that patients get the follow-ups needed for future treatment.
Communicates with case management and the hospitalist group to ensure Medi-Cal patients' discharge needs and allowances required by their insurers have been addressed from the patient's admission rather than at discharge
