Here, you will make a difference each and every day as you work alongside a supportive team.As a Community Liaison, you will impact the lives of our patients by -Analyze the potential of the company's service are to determine target marketsVisit doctor offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities and other possible sources of referrals to present agency credentials and obtain patient referralsAnalyze the company's organization to determine its strengths and weaknessesAnalyze past and current marketing dataComplete and analysis of the company's service lineAnalyze patient/company relationshipsDevelop a marketing plan, identifying priorities and sets a reasonable timelineImplement marketing plan staying within established timelineReview and evaluated the analysis and plan on an established basisProvide coordination of referral through all Dover Health servicesAttend IDG meeting, as appropriateServe as a first contact from clinical referral sourcesProvide initial informational meetings with patients and/or familiesAssist office staff as needed with obtaining signatures and updatesProvide community outreach and educationParticipate in discharge planning, as needed.Assess patient's/family members' ability to cope with the patient's dyingFunction as consultant to the members of the health team, assists them in understanding the social, emotional and environmental factors related to the patient's health problemsHelp patients to utilize the resources to their families and the communityTo become a Dover Health Community Liaison, you will need -At least 1 year experience in marketing, nursing or social work interacting with health agencies.Clinical license and experience preferredHospice or home care experience preferredMust be a licensed driver with an insured automobile in good working order, and clear driving record.
