OverviewMHealth Fairview has an immediate opening for a Clinical Dietitian at our Kasota location located in Minneapolis. This benefits eligible position is a 0.6FTE (48 hours/ 2weeks) on the day shift.The Clinical Dietitian is responsible for nutritional assessments, care planning, implementation, and evaluation of goals/outcomes for FHI RD-monitored patients receiving enteral feedings and TPN, and for other FHI patients as consulted. Performs all responsibilities in accordance with policy, procedure, and professional practice guidelines. Coordinates nutrition care with other members of the health care team and delegates responsibilities to others as appropriate. Provides nutrition education to patients, families, other health care professionals. Promotes the growth and development of the Home Infusion Enteral Program and Nutrition Support Team Responsibilities Job DescriptionPerforms nutrition assessments on FHI RD-monitored patients receiving tube feedings and TPN, and on other patients as consulted. Documents actions according to FHI guidelines.Collaborates with patient, family, and other healthcare team members in developing the plan of care. Individualizes plan based on age, culture, social considerations, and other relevant needs. Determines nutrition interventions based on patient needs and expected outcomes. Documents actions according to FHI guidelines.Provides care according to the plan and in accordance with all relevant policies, procedures, and standards of care. Coordinates nutrition care with other healthcare team members. Delegate's nutrition and direct patient care activities to other healthcare team members as appropriate. Documents actions according to FHI guidelines.Reassesses nutrition status and effectiveness of interventions at specified intervals. Evaluates response to interventions. Modifies the interventions based on goals/outcomes.Serves as a resource to customers, staff, and physicians regarding Fairview Home Infusion services. Promotes the growth and development of the FHI Enteral Program and Nutrition Support Team. Writes policies and procedures for nutrition support that are consistent with national standards.Provides Patient/Family Education. Considers age, education, and socio-economic data.
