We are looking for a Registered Nurse to join our team in and around the following towns: Antioch, Waukegan, North Chicago, Lake Forest, Highland Park, Lincolnshire, Vernon Hills, Libertyville, and Grayslake.Benefits:Competitive Pay$2,500 sign-on bonusFlexible ScheduleGreat work-life balanceSalary OfferedPTOMedical Insurance401K, Vision, DentalQualifications and Responsibilities:Observation, assessment, and documentation of patient symptoms and progressPerform initial home care visit and re-evaluate patient needs and progress when neededDemonstrate efficient teamwork with the staffDemonstrate competence in documenting OasisSupervise HHA field staff as indicatedFollow Medicare regulationsNotify physician of change in patient's conditionCoordinate other disciplines as applicable to assist patientComplete paperwork timely and efficientlyEducate patients and caregivers on disease process, medications, plan of care, and treatment plans according to the policy and procedure manualProvide quality of care to patients in their homes according to policy and proceduresProvide wound care and assessment. Provide IV line care and maintenance
