This position will also provide support for the Employee Health process overseen by the Infection Control Manager. Case Management responsibilities will include supporting the Case Manager in planning and coordinating patient care. This will include assisting in coordinating and delivery of needed services, and providing education to patients and their families, and collecting required paperwork.QUALIFICATIONSOne to two years of quality assurance functions or nursing case management previous experience required.Active/Current TX RN licenseDemonstrated skill in organizational theories, management techniques, methods, theories and principlesExcellent communication skills including a high degree of professionalism required.Basic knowledge of clinical patient care required.Basic understanding of infection control principles required.Basic knowledge of process improvement and system scienceWorking knowledge of MS Office required.Ability to make decisions when data/files require a higher level of review required.Ability to identify potential high risk compliance issues in risk management/infection control RESPONSIBILITIES:Tracks and analyzes clinical/operational opportunities. Assists stakeholders with program planning and decision making around use of data.Supports public reporting and regulatory data.Assist in the risk management process, adhering to the risk management plan.Assists in the facilitation of ongoing monitoring/surveillance of infection prevention.Collects, analyzes and trends FPPE/OPPE data.Assist in the oversight of the Performance Improvement program.Ensures appropriate reporting of adverse events to Quality Manager.Maintains current knowledge related to guidelines and standards necessary to perform job duties. Infection Control-responsibilities include infection control performance improvement project and to collect and analyze infection control and surveillance of data.Performs continuous assessments and evaluations to ensure the patient is progressing towards desired outcomes.Assesses and responds to patient/family needs by coordinating efforts of other team members.Identifies and resolves barriers that may hinder patient's progress toward established goals.Collaborates and consults with physicians on patient's progress and discharge planning needs.PHYSICAL DEMANDSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
