Key Responsibilities: Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician. Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff. Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means. Minimize patient risk by front-loading visits for high-risk patients and teaching them preventative measures. Counsel patients and family members on disease/injury management and proper use of equipment. Supervise therapy services provided by PTAs and nurse assistants. Communicate with PTAs and nurse aides on supervisory visit results. Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Understand and follow agency policies, procedures, rules, and regulations. Requirements: Physical Therapist with a current license in the state of employment. Minimum of one year of experience as an PT in an acute setting. Valid driver's license and auto insurance in your name as a driver. Capable of all physical demands.
