JOB SUMMARY

A Care Coordinator helps primary care patients of Curry Health Network, who live in their community, lead happier, healthier lives.By helping with the coordination of care, they assist patients with complex conditions and manage office visits, testing services and treatments. They often work with vulnerable populations, ensuring that they have access to care and programs that can help them lead healthier, safer and more secure lives. Their educational role also aids in the prevention of illnesses and disease to benefit the entire community and reduce burdens on the health care system.Their overall goal is to help patient have better health outcomes.

ESSENTIAL FUNCTIONS

  • Coordinating care between numerous healthcare providers
  • Referring patients to providers and community resources who can assist them
  • Sharing health and wellness tips relevant to patients
  • Advocating for patients attempting to make use of public health and government programs
  • Explaining health information in a culturally sensitive, easy-to-understand manner
  • Maintaining accurate records of all patient contacts
  • Protecting patient confidentiality

MINIMUM JOB REQUIREMENTS

Education & Experience
  • High school diploma.
  • Completion of an approved training program within 12 months of hire.
  • Be recognized by the Oregon Health Authority's Traditional Healthcare Worker registry within 18 months of hire.
Required Knowledge, Skills & Abilities
  • Proficient computer skill including Microsoft Office (Word, Excel, Outlook).
  • Strong interpersonal skills with an ability to work with people in close situations, using excellent communication and listening skills.
  • Strong oral communication skills to convey health information in an easy-to-understand manner.
  • Strong organizational and prioritization ability.
  • Multitasking and time management is critical to performance of the role.
  • Understands medical terminology, human anatomy and common conditions and treatments.
  • Problem solving ability to help overcome obstacles to care.
