Position Summary The RN Case Manager (RNCM) is responsible for ensuring terminally ill patients receive exceptional end?of?life care. In this role, the RNCM works collaboratively with an interdisciplinary team to provide end?of?life care to patients needing hospice services in their home or community setting. The RNCM provides individualized case management, admission facilitation, clinical support, continuous care, after?hours call, and other skilled nursing expertise. Salary $83,000-92,000 annually Job Requirements Succinctly and concisely convey patient assessments with clinical data to hospice physician who will then determine hospice eligibility Provide direct patient care to terminally ill patients; assess, identify and address their individual physical, psychosocial, safety, and comfort needs Anticipate, prevent and treat undesirable symptoms or secondary symptoms Develop, maintain, and manage an individualized care plan that captures the evolving needs of the terminally ill patient Train, instruct and support the patient, family and caregiver Document patient problems, nursing assessments, patient goals, care provided and patient/family outcomes from interventions and care provided in a timely manner Consult and collaborate respectfully with the interdisciplinary team and others involved in the patients care Prepare and present all needed information for bi?weekly IDG meetings Qualifications Graduate of accredited school of nursing Current state of Minnesota RN licensure Three years recent nursing experience in acute, long?term care or home care/hospice/public health Patient care planning and patient teaching skills Written and verbal communication skills Critical thinker Current Minnesota drivers license, insurance and daily access to reliable vehicle Computer literacy Benefits Medical insurance Dental insurance Vision insurance Long/short?term disability Basic life/AD&D Supplemental Life/AD&D Dependent Life/AD&D Paid vacation and sick days and 11 paid holidays Flexible spending accounts for healthcare and dependent care reimbursement Being part of a caring and dynamic team About Grace Hospice Grace Hospice care encircles you with comfort, pain management, honesty, and respect for your decisions.
