Performs therapy and communicates sufficient information about the treatment plan to nursing and other personnel to assure a team approach. Documents therapy services and writes progress notes in accordance with Medicare Regulations.When low census, must be willing to travel within a 30 mile radius to provide therapy coverage. (Mileage/time reimbursed). If a therapist is hired as a Float therapist, all Float therapists can be asked to travel to any LifeSpan/Cardon community (travel time reimbursed). CarDon Associates is an Indiana-based and family-owned provider of senior living options and lifestyle support services, ranging from independent living and assisted living to skilled nursing and memory care.
