Position Overview Regional Hospice is looking for a FULL TIME RN Case Manager for our Home Hospice patients. The role requires a 40?hour day shift. Benefits include Vision, Medical, Life, Dental, 401K Match, Vacation/Sick Time, and use of a vehicle for work and personal use. The Case Manager coordinates and manages all patient care and other services for patients and their families, working with an interdisciplinary team and the patients primary care physician to ensure quality pain and symptom management and support for the patient and family. Responsibilities Coordinate and manage all patient care and services. Work with interdisciplinary team and primary care physician to provide quality pain and symptom management. Support patients and families throughout care. Qualifications Registered Nurse with a current license in the State of Connecticut. At least two (2) years of staff nurse experience in a hospital, home health care agency, or hospice. Advanced credentials in hospice care preferred. Valid auto insurance and clean driving record; transportation required. Excellent verbal, written, and computer skills. Bachelor of Science in Nursing preferred. Good communication and organizational skills; professional attitude and appearance. Strict confidentiality of patient and agency information.
