Job Description:

This position will work in both inpatient and outpatient settings. Screenspatients for nutritional problems as identified by nurses admit nutritionalscreen, patient referral by a member of the health care team, or familyrequest. Implements the nutrition careprocess for patients with nutrition diagnosis. Fosters continuity of careacross the continuum of care. Providesnutrition education to patient and family members on appropriate foodselections and eating behaviors to accomplish patient goals. Participates in performance improvementactivities. Supervises diet clerks andeducates staff regarding therapeutic diets. Acts as liaison between thepatient and the kitchen. Participates inthe daily patient rounds (care conference). Provides individual and group outpatient nutrition education andMNT. Performs the duties of instructorfor the nutrition, physical activity and mental health aspects of the groupdiabetes education program. Actively assesses, evaluates and maintainsprofessional development and competency related to the competencies outlined inthe position description.
