MEDICAL COUNSELOR

  • 40 hours per week (Mon - Fri 6am-2pm)
  • The pay rate is $19.75 per hour
You must have a valid NY State driver's license and a HS diploma/GED to be considered for this position. The individuals at this location need some ADL supports as they have moderate to low capabilities. Lifting adult individuals is necessary using a 2 person lift and equipment. Qualifications include:
  • High School Diploma or equivalent
  • Prior experience working with persons with developmental disabilities preferred
  • Minimum six months' experience with Life's WORC or similar agency in capacity of Support Counselor or in another health related field
  • Excellent verbal and written communication skills
  • Valid NY State Driver's License required
Responsibilities Include:
  • Accompany and advocate for individuals on medical/dental appointments and emergency room visits
  • Schedule all medical appointment in coordination with the Nurse and Manager
  • Maintenance of a medical appointment calendar in coordination with the Nurse and Manager
  • Meets with the Nurse and Manger weekly to coordinate information brought to and from appointments
  • Provide ongoing protective oversight to individuals living in the residence
  • Implementation of active treatment
  • Provide assistance and instruction to each individuals in basic activities of daily living
  • Provide training and support for each individuals in accordance with the individual program plan
  • Perform daily check of medication and supplies
  • Successful completion of the AMAP course and annual recertification for administration purposes
  • Scheduling flexibility as needed; may be required to work beyond the normal shift
  • Drive agency vans that are equipped with GPS monitoring systems and demonstrate safe driving practices
  • Assist in maintaining a clean and safe environment; proper use and maintenance of household equipment and appliances
