Element Care is seeking a compassionate and dedicated Personal Care Specialist to join our team. This position is critical in fulfilling our commitment to providing the highest quality of care to elderly patients who are part of our PACE program. As a Personal Care Specialist, you will work closely with multidisciplinary teams to offer comprehensive in-home services that enable seniors to live independently and with dignity. Provide daily living assistance to the elderly, such as bathing, dressing, and grooming. Monitor and report changes in patient health status. Coordinate with healthcare professionals to implement personalized care plans. Assist with the administration of medications as prescribed by the healthcare provider. Facilitate recreational and therapeutic activities that enhance the quality of life for patients. Maintain a safe and clean environment for all patients. Educate patients and their families on proper home healthcare strategies and techniques. Strong interpersonal and communication skills. Ability to work independently with minimal supervision. Proficiency in maintaining and handling personal care equipment and products. Capability to perform physical tasks frequently, such as lifting and moving patients.
