Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network works together to create opportunities for older Ohioans to receive needed home and community services and supports; and to age successfully in their own homes and communities.

Position Overview;

The primary role of the Medicaid Navigator is to assist high risk individuals with obtaining Medicaid benefits required to facilitate enrollment onto Medicaid Waivers administered by the Ohio Departments of Aging and Medicaid. Join us in making a meaningful difference in healthcare accessibility for vunerable populations.

Primary duties include:
  • Serve as a liaison between WRAAA and County Departments of Job and Family Services (CDJFS) regarding verifications for community programs and Medicaid
  • Navigate within the OBWP (Ohio Benefits Work Portal), MITS (Medicaid Information Technology System), and PASSPORT Information Management System (PIMS) to receive, retrieve, investigate and communicate information regarding Medicaid eligibility.
  • Communicate with assessed individuals to provide guidance and education on enrollment status, verifications needed for programs and the Medicaid process.
  • Complete in-home visits to obtain and scan verifications to the applicable county JFS.
  • Complete long term care denials in the OB system for applicants assessed for but not pursuing enrollment onto PASSPORT and Assisted Living Waivers.
  • Provide coverage of ADRC related support activities that facilitate enrollment including:
    • Assist with obtaining physician certification
    • Assist with scheduling assessments
    • Assist with processing daily waiver enrollments
  • Make immediate referrals to emergency responders when the individual is in a crisis or in an emergency; conduct follow up to ensure needed assistance was received.
  • Connect individuals to community resources when needs are identified.
  • Maintain confidentiality according to the Agency policy.
  • Participate in training, meetings, in-services, volunteer events, and continuing education opportunities for professional development or as requested by supervisor, manager or director.
  • Participate in and contribute towards accomplishments of WRAAA performance goals.
  • Promote positive employee relations and fosters excellence in customer service

Requirements:
  • Associate's degree in human services, health sciences or related field with at least 2 years of experience, OR Certified Community Health Worker with at least 3 years of experience. Preference will be given to bachelor degreed applicants.
  • Must have experience working with individuals regarding Medicaid eligibility requirements and long-term service and support programs and resources.
  • Excellent customer service skills with the ability to effectively communicate with the public, both in person, by phone and email.
  • Ability to accurately document facts related to essential job functions within required timeframes.
  • Ability to perform data development, collection, collation and evaluation.
  • The ability to independently solve problems including the ability to analyze, evaluate and implement reasonable courses of action based on available information.
  • Ability to travel to community and consumer locations within our five-county service area (Cuyahoga, Lake, Lorain, Medina and Geauga counties).
  • Ability to quickly and effectively navigate MS applications including Excel, Word, SharePoint, Teams, Access, Outlook and the ability to learn and effectively navigate job related IT platforms.
