Job Description

Seeking a dedicated Intake Nurse to join our large hospice organization. Responsibilities include coordinating referrals, verifying and entering physician's orders, determining the appropriate level of care for patients, reviewing documentation for facility placement, and coordinating orders with pharmacists. The role demands close collaboration with referring hospitals, physicians, skilled nursing facilities, patients, and their families.

Hard Skills

+ LPN or RN license

+ 1 year experience in a skilled nursing facility

+ BLS certification

Soft Skills

+ Excellent communication skills

+ Strong organizational skills

+ Ability to work closely with various healthcare professionals

Work Site

This is a fully on-site position in Woods Cross, Utah.

Work Environment

The position operates on a 4/10 work schedule, with every other weekend off, and consistent off days every Monday and Tuesday. The work environment is business casual in an office setting, with a team of 9 immediate colleagues out of 32 in the office
