Priority Healthcare Group is seeking a Clinical Liaison for our Skilled Nursing Facility clients ( Orangeville, Millville, Mt Carmel, Shenandoah) The Clinical Liaison is responsible for managing patient referrals in order to ensure a smooth transition.
Responsibilities:
•Clinical assessment of electronic medical charts from hospitals to determine if a facility can meet the clinical needs.
•Verification of benefits
•Obtain pre-authorizations from insurance company
•Communicate clinical information to facility
•Ensure closure with each referral
Qualifications:
•RN/LPN/LSW, or
•Over 2 years' experience in nursing home industry in one of the following roles: Central admissions, Director of Admissions, Clinical Liaison, Director of Marketing, or Social Services
