LPN Rehab and Nursing Float Pool

About the Department The CHCC Float Pool encompasses offers flexible staffing positions to support the staffing needs of our Rehab and Nursing Centers in West Michigan.
This includes Kentridge, Cedar, and Leffingwell, Greenville and Reed City.
Team members will be allocated to a unit prior to their shift and will be preassigned in advance.
Team members will have a home entity.
If they are not needed in at any other entity, they will be assigned to their home entity and go into rotation for floating per the existing departments algorithm.
Compensation There is additional Float Pool compensation based on where you are willing to travel.
For those willing to travel to all 5 of the locations, they will receive additional $15.00 an hour compensation.
For those willing to travel to all 3 Grand Rapids locations and 1 additional regional location, they will receive $10.00 an hour compensation.
Those willing to only travel within the Grand Rapids locations, they will receive an additional $5.00 an hour compensation.
Rehab and Nursing Locations Kentridge We are located on a 10-acre campus. Our indoor and outdoor spaces are designed to provide opportunities for improving independence, strength, and confidence. Two main floor courtyards provide outdoor space for rest and renewal. Most patient rooms are private, and have an attached bathroom and shower, television with cable, phone, and a large window. Each unit has a large and accessible living and dining space, and a menu-based meal service is provided.
Leffingwell The Rehab and Nursing Center – Leffingwell was designed with healing in mind. The center offers 80 private rooms with beautiful nature views. In addition to a dedicated interdisciplinary team, physical, occupational, recreation and speech therapists will provide patients with a customized therapy program designed to meet individual rehabilitation needs. Our expert team of therapists will use the latest technology and techniques to maximize independence and mobility to help prepare patients to return home.
Cedar The Rehabilitation & Nursing Center – Cedar was designed with the word “home” in mind. The home is more spacious than the previous 52-year-old Fuller Street facility and boasts many new amenities, including four spacious interior courtyards; a new dining room; on-site dialysis den; art room, private and semiprivate rooms with their own window, restroom and shower; modern furnishings; and new technology, to make these community members feel at home.
Scope of work Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care. Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician. Documents care provided via computer/written patient record in a manner consistent with guidelines.
Qualifications Licensed Practical Nurse(LPN) Associates in Nursing Required MI Licensed Practical Nurse (LPN) Required AHA or ARC Basic Life Support (BLS) Required within 90 days
