The

Afterhours RN in our Advanced Illness Management (AIM) program

will report directly to the Director of AIM. Responsible for identifying and coordinating patient/family needs and for providing preventive and supportive care to the palliative patients and family. Utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life. Assumes afterhours coverage for the designated teams and functions as an interdisciplinary advanced illness management and support, along with telephonic, video, and in person interventions afterhours.

As a Registered Nurse / RN, you will:
Be able to triage patient needs over the phone until a nurse can provide an in-person visit
Respond appropriately and with urgency to crisis calls from patients and families after hours
Make the initial nursing evaluation in determining eligibility for palliative serves, as part of a patient visit, within 48 hours of referral
Identify the patient/family's physical, psychosocial, emotional, and environmental needs and re-assesses as needed
Collaborate with the patient/family, attending physician and other members of IDG in providing patient and family care
Timely document the problems, appropriate goals, interventions, and patient/family response to palliative care
Maintain accurate and relevant clinical notes regarding the patient's condition
Be responsible to effectively communicate with members of the AIM team, clinical supervisor, and the patient's PCP, if any, regarding changes in patient status and/or patient needs occurring during afterhours call
Report patient condition and any identified needs/issues to supervisor, nurse practitioner, or PCP
Assist in overseeing the utilization of medications, DME and supplies related to the plan of care in a manner consistent with the needs of the patent, physician orders, and the performance goals of the AIM agency

About You:

Registered Nurse / RN Requirements:
Education:
Associates/Bachelor's degree Nursing

Experience:
2 years of prior skilled Registered Nurse experience
Demonstrated ability to provide quality patient care
Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
Additional, preferred experience to help you be successful in a hospice role: hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing

Licenses, Certifications and/or Registration:
Active and unrestrictive RN license to practice in the state without any board action
CPR Certification

Equipment/Tools/Work-Aids:
Fluent competence in computer programs Microsoft Outlook, Microsoft Word, and Microsoft Excel
Fluent in electronic medical record documentation

Specialized Knowledge and Skills:
Prior clinical experience
Strong compassionate phone skills
Strives for independence and sound critical decision-making regarding the delivery of patient care
Competent to perform afterhours in person visits
Familiar with the concepts and needs of patients/families who are facing advanced illnesses
Excellent written and verbal communication
Always maintains patient confidentiality
Good problem-solving skills
Good time management skills

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity.

Salary Range – $66,890-$78,645
