for providing health education and instruction in the areas of e.g., health/sickness, personal hygiene, nutrition, sexual behaviors, and medication usage as applicable. Responsible for meeting and consulting with other staff providers on an on-going, regular basis to discuss clinical concerns and opportunities for care improvement. Additionally, consults with physician on duty as necessary and refer patients to appropriate resources. for providing medical services that meet standards of care, and agency policy, procedures, and protocols. Responsible for participating in The Joint Commission (TJC) PCMH daily morning huddles.Responsible for performing patient triage to include direct patient assessment, patient treatment and, consults with the medical team throughout triage and refers patients to the medical team for treatment as needed. Responsible for actively participating in the agency's Improving Organization Performance Program to include e.g., Infection Control, Safety and Emergency Preparedness Programs and the TJC.REQUIREMENTS:Current California Registered Nurse license Masters Degree from an approved accredited University School of Nursing (completion of a program of training which conforms to standards established by the California Board of Registered Nurses for Nurse Practitioners Current Nurse Practitioner Certification and two years of Nurse Practitioner experience Current board certification Current DEA certificate Current CPR certificateBilingual (English/Spanish) Valid California Driver's license and California basic automobile insurance Knowledge of electronic medical records Ability to communicate effectively both verbal and written communication
