Southwest Medical, part of OptumCare, is a multi-specialty group of physicians, Nurse Practitioners, and Physician Assistants consisting of over 300 providers, 21 medical health centers including six urgent care clinics, four convenient care centers, two lifestyle centers catering to older adults, an outpatient surgery center, home health, hospice and palliative care services, plus E-visits and online options via Now Clinic telemedicine technology. Southwest Medical offers patients compassion, innovation, and quality care throughout southern Nevada. Southwest Medical is headquartered in the greater Las Vegas, NV area.

Primary Responsibilities:

Telephonic LPN position

Review medical charts to determine if patient follow up needed

Contact patients and make sure hospital follow up appointment is scheduled

Track patient follow up appointment status

Coordinate with and support RNs

Review daily reports , identify patients applicable for out-patient case management and create referrals

Telephonic position with occasional face to face patient contact in SMA clinics

Identify gaps in patient care

resulting from lack of HH f/u, DME not being delivered etc.

Coordinate with multiple SMA/Optum departments throughout the continuum to ensure timely care coordination

Create reports to quantify departmental metrics for all staff members

Excellent customer service

Ability to adapt to change

Other responsibilities as assigned

High school diploma or GED

Current, unrestricted NV LPN license or temporary NV LPN license with the ability to obtain permanent NV LPN license upon expiration of temporary NV LPN license

CPR certification or willing to obtain within 30 days of hire

2+ years of clinical nursing experience required (preferably in an acute care or clinic setting)

Experience using EMR

Microsoft Office proficiency (Word, Excel)

The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
