**Introduction**:
The Department of Health Services (DHS), **Mendota Mental Health Institute (MMHI)** is currently recruiting for a Rehabilitation Case Coordinator.

MMHI is located at 600 Williamson Street in Madison. 600 Williamson Street, Suites A anMadison, WIMMHI is an inpatient psychiatric facility, with one outpatient program, specializing in serving patients with complex psychiatric conditions, often combined with certain problem behaviors. A secure setting to meet the legal and behavioral needs of our patients is provided. MMHI is an integral part of the entire Wisconsin mental health service delivery system.

**Position Summary**:
As the Rehabilitation Case Coordinator, you are responsible for developing, coordinating and implementing comprehensive clinical plans for clients in the Program of Assertive Community Treatment (PACT) Program. Responsibilities include:

- provision of academic and vocational rehabilitation services
- provision of direct clinical services
- consultation and technical assistance to other PACT staff on employment/rehabilitation issues and participation on various treatment teams in a vocational capacity
- development of a full range of vocational and independent living options in community settings
- consultation and training

**Salary Information**:
The starting pay is between $28.50 and $30.50 per hour (approximately between $59,200 and $63,400 per year), depending on qualifications, plus excellent benefits. A 12-month probationary period is required.

**Job Details**:
All applicants who may be appointed to this position will be required to allow DHS to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled.

All appointees will be required to receive a TB test, which will be administered by the facility or medical personnel directed by the facility within 30 days of the start date.

**Qualifications**:
Minimally qualified applicants will have all of the following:

- Meet the qualifications for clinical coordinators under **WI Administrative Code DHS 63.06(2)(c) or (4)(a) 1-8** at the time of appointment.

Well-qualified applicants will also have one or more of the following:

- Experience completing clinical and vocational behavioral assessments.
- Experience providing vocational rehabilitation services and community treatment for persons with long-term mental illness.
- Experience job coaching and assisting individuals who are seeking employment.
