**Position Overview**: As a Registered Dietitian at Tiburon Hills Care Center, you will play a vital role in enhancing the health and well-being of our residents. You will be responsible for assessing nutritional needs, developing personalized meal plans, and providing nutrition education to residents and their families. Additionally, you will collaborate with our interdisciplinary team to ensure that each resident receives the highest quality of care.

**Key Responsibilities**:

- Conduct nutrition assessments and develop individualized care plans.
- Monitor residents' nutritional status and make appropriate recommendations.
- Provide nutrition education to residents, families, and staff members.
- Collaborate with the interdisciplinary team to coordinate care and ensure optimal outcomes.
- Maintain accurate documentation of nutritional assessments and interventions.

**Qualifications**:

- Bachelor's or Master's degree in Nutrition or Dietetics.
- Current registration as a Dietitian with the Commission on Dietetic Registration.
- Experience working in a long-term care or healthcare setting preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively as part of a multidisciplinary team.
