**Responsibilities**:

- **Home Health experience and an RN license are mandatory prerequisites.**:

- This position is equivalent to a HH Team Manager or DON.
- The Home Health DPCS is responsible, as the oversight manager, for some field clinical and some office staff. Possessing primarily Home Health experience overseeing an ADC of 250+ patients will definitely put you in the front of the line
- Reporting to the Administrator, you will be responsible for the implementation, delivery, coordination, with managerial oversight, of the clinical and administrative home health services delivered by your team and regional IDT’s consistent with best practices, agency policy and procedures.
- Participates and provides leadership for the team during regular weekly IDT meetings.
- Reviews and revises the patient/family care plans, as needed.
- The Home Health DPCS serves as a member of the clinical leadership team in order to address problems associated with the delivery of clinical services, identifies staff development needs and puts forth suggestions for appropriate changes in policies and procedures.
- Supervises the maintenance of medical records ensuring accuracy, completeness and compliance with regulations, legal and ethical standards and team meets compliance standards and team meets compliance standards and team meets compliance standards on quarterly and random clinical record audits.
- Collects and reviews pertinent clinical data at team meetings.
- Submits Plans of Correction to Administrator when required by audit results and/or patient family satisfaction surveys.
- Monitors documentation of all team members, and counsels IDT when documents do not meet company standards.
- The Home Health DPCS conducts supervisory visits to allow for observation of performance of all team members.
- Provides a number of customer in-services to each contracted facility required by law, and provides appropriate supporting documentation.
- The Home Health DPCS arranges team coverage for on-call and staff time off.
- Provides leadership and counsel to regional personnel to enhance staff development.
- Meets budgeted Average Daily Census and Admissions goals.
- Meets referral conversion goals as set by the Administrator.
- Engages with regional community education liaison regarding business development and displays commitment to the growth of the region and organization.

**Qualifications**:

- Bachelor’s Degree in Nursing is preferred, or an ASN with commensurate clinical experience.
- Home Health RN supervisory experience is a mandatory prerequisite.
- Must have a current or renewable CA RN license.
- 5+ years experience as a Home Health RN is much preferred.
- CHPN certification is a plus
- Must have a valid Driver’s license and reliable transportation.
- Strong verbal and written communication skills.
