**A little more about our center**:
Community Care Center is a large inner-city Skilled Nursing Facility. We have patients of all ages with multiple levels of care, including long term care and a large behavioral health (psych) population.
- We have a new Dialysis program coming in 2024
- The average age of our patients is mid-40s
- Along with our medical/skilled patients, the most common diagnoses of the rest of our population is schizophrenia, bi-polar and major depressive disorders.
- The Staff and patients are welcoming and say hello as you walk through the hallways.
- We are located near W 43 ST & State Street, just off 90/94 and a few blocks from the train station.

The **_National Association of Rehab_** Providers & **_Agencies_** (**_NARA_**) awarded Transitional Care Management their **“Impact Award” for 2023** for our Life Skills Programming. **Job Description** What you will do in this role**:
As a Registered Nurse (RN), you will be joining a team of nursing professionals dedicated to providing excellent clinical care. Some have been with us for a long time and can mentor others. You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team.

**Qualifications** Floor Nurse**: We will consider a Registered Nurse (RN) for this role.

**What we need from you**:

- Registered Nurse (RN) in IL
- Prior floor nurse experience within a skilled nursing facility is helpful
- Experience with the post-acute patients - specifically the cardiac, post op ortho & the medically complex - is ideal
- Electronic medical records experience is helpful (Point Click Care or PCC is ideal)
- New Grads are encouraged to apply
