The primary purpose of this job title is to plan, organize, develop, and direct the MDS schedules and the Care Plan meetings in accordance with current/applicable Federal, State and Local Standards, Guidelines, and Regulations as may be directed by the Administrator to assure timely, complete and accurate MDS/RAPS/CARE PLANS resulting in Quality Resident Care.

**Responsibilities of MDS Coordinator/RNAC**:

- Accurate and thorough completion of the Minimum Data Set, Care Area Assessments and Care Plans, in accordance with PDPM and current federal and state regulations and guidelines that govern the process
- Proactively communicates with Administrator to identify regulatory risk, clinical trends that impacts resident care, and any additional information that has an effect on the clinical and operational outcomes of the center
- Collaborates with the facility’s staff to select the correct assessment of our residents and acts as an in-house Case Manager
- Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Managed Care and Medicaid
- Ensures timely submission of all Minimum Data Sets to the state data base and ensures that the necessary follow-up action is taken
- Facilitates the Care Management Process by engaging the resident and all other appropriates staff members
- Analyzes QI/QM data with the Director of Nursing to identify trends on a monthly basis and serves on, participates in, and attends various committees of the facility.

Qualifications:

- Registered Nurse in the state Georgia
- Bachelors Degree
- Preferred 1YR experience working as MDS Coordinator
