Hospice Lvn

We are looking to hire an LVN to assist with our hospice patient visits. Working on a full-time basis, the nurse will work with a team of healthcare providers to ensure our hospice patients are receiving quality care. The job will serve our patients primarily in the south I-10 areas.

**Responsibilities**:

- Conducting routine visits, admissions visits and/or follow-up visits at home or other facilities settings.
- Coordinating, assessing, and implementing patients' care.
- Providing accurate and timely documentation of patient services to reflect plans of care.
- Flexibility to be available days, evenings and weekends.

**Qualifications**:

- LVN license (current, unrestricted) in the State of Texas
- Hospice experience _required_
