**Bilingual Outreach Case Manager
- Respite Program**

**SUMMARY**: We are seeking a dedicated and compassionate Bilingual Outreach Case Manager to join our team and make a significant impact on the lives of seniors and their caregivers by connecting them with essential respite services. In this role, you will have the opportunity to engage with clients, develop effective outreach strategies, and cultivate community partnerships to promote respite care services. **KEY RESPONSIBILITIES**:

- Manage and coordinate outreach activities, including community events and presentations.
- Represent the organization at events, emphasizing the benefits of our respite care services.
- Cultivate and maintain partnerships to expand the program's reach.
- Effectively communicate the program's mission and services to recruit respite providers.
- Collaborate with seniors to establish personalized goals, conduct regular check-ins, and assess their needs.
- Monitor seniors' well-being on a weekly basis and adjust support as necessary.
- Track and report performance metrics to measure outreach effectiveness.
- Analyze data to inform program adjustments and improvements.
- Organize and maintain multiple cases concurrently, ensuring thorough and efficient support.
- Build professional relationships with coworkers, vendors, and the community to enhance program effectiveness.

**MINIMUM QUALIFICATIONS**:

- Bilingual (Spanish/English).
- Bachelor’s degree in in social work, communication, marketing, business, or a related field (preferred).
- 2-5 years of experience in conducting outreach, community engagement, and case management.
- Exceptional written communication and presentation skills.
- Proficiency in preparing reports and analyzing data.
- Competency with computers and MS Office Suite.
- Strong ability to work with diverse populations, demonstrating compassion, adaptability, and cultural competence.

**REQUIREMENTS**:

- Must be able to receive clearance through a fingerprint/background check (criminal and child abuse).
- Possession of a valid California driver's license, a clear DMV record, current automobile registration, and an insured automobile in good working condition.
- Vaccination for COVID-19 required.

**SALARY**: The starting salary range for this position is $45,000-$50,000 per year DOE. Family Support Services also offers excellent benefits: Employer-paid Health, dental, life insurance, and longterm disability.
