**Responsibilities**:

- **Patient Assessment**:

- Monitor and manage symptoms effectively to ensure optimal comfort for patients.
- **Continuous Care**:

- Provide continuous nursing care during periods of crisis, ensuring patients receive appropriate attention and interventions.
- Collaborate with the interdisciplinary team to adjust care plans based on evolving patient needs.
- **Medication Management**:

- Administer medications as prescribed and monitor for effectiveness and side effects.
- Educate patients and their families on medication management, emphasizing the importance of adherence to the care plan.
- **Communication and Collaboration**:

**Qualifications**:

- LVN with a current license in CA.
- Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Compassionate and empathetic approach to patient care.
- Knowledge of hospice regulations and best practices.

**Working Conditions**:

- This position involves working in various settings, including patients' homes and healthcare facilities.
- Flexible schedule, including occasional evenings and weekends, to meet patient and family needs.
