**Starting salary is dependent upon qualifications, but in most cases is no higher than the midpoint for range.**

**NATURE OF WORK**:
Work involves the development and coordination of Health and Human Services projects, programs and services with other City staff and community agencies. Work involves coordinating the service delivery of a variety of programs and projects within the community under the direction of the Human Services Manager, many of which may be of a short-term nature. The work includes professional, administrative, managerial and hands-on responsibilities to accomplish the goals and objectives of the Department and the specific program or project. Works with a variety of City staff and community agency representatives, and must be able to collaborate successfully to bring about the desired health and human service outcomes. Working schedule varies, and is designed to accommodate the projects or programs being coordinated, and may involve weekend and evening hours.
**ESSENTIAL FUNCTIONS (Specific assignment will include some or all of the following)**:

- Work in collaboration with community partners to maximize community engagement activities and implement programming on health, wellness, disease prevention, and social determinants of health.
- Develop, design, and implement health education and community health improvement programming tailored to community/ subpopulation needs.
- Foster cross-sectoral community partnerships to address community health needs.
- Articulate connection between health needs and root causes of health inequity/ social determinants of health.
- Exhibit substantial cultural competence and knowledge by ensuring various demographics receive health education on topics vitally important to their particular needs and circumstances.
- Performs outreach, planning and organizing health education and human service programs that increase community awareness of services within the community.
- Support development of desirable health and human service goals and determine availability of professional health services.
- Research evidence-based community health interventions to inform program and service delivery.
- Survey regulated community health operations and facilities to determine compliance with applicable codes.
- Review and analyze the community, human service and public health needs assessment process results in terms of coordinating the delivery of programs and activities identified.
- Support the health department community health assessment process, assisting in collecting, analyzing, and visualizing quantitative and qualitative data, in coordination with Department Head and Management Analyst.
- May investigate public health and human service complaints and provide education-based resolution of public health-based conflicts amongst individuals and/or groups within the community.
- Spearhead health promotion programs such as We’re Out Walking(WOW),Rethink Your Drink, Tobacco Cessation initiatives and other health activities
- Develop marketing, communication, publicity and instructional material for use in programs and services including workshops and lectures.
- Meet with community health providers and consumers regarding community health and human service needs, representing the Department.
- Coordinate cross-functional health and human service program services to meet community health goals.
- Maintain informational databases and dashboards including information on the webpage for human services and health operations.
- Prepares and disseminates educational and informational health and human service materials for community groups
- May perform communicable disease contact traceback and notification
- Attends various community meetings, board and Council meetings, preparing and presenting information as needed.
- Performs other duties as assigned.

**MINIMUM REQUIREMENTS OF WORK**:

- Possession of a Bachelor’s degree in public health, health administration or related field, and at least 2 years of experience in the provision of public health and human service program services in a municipal setting or community health setting. Possession of a Health Education Specialist certificate (CHES) is preferred or the ability to obtain the certificate within 2 years of hire.

**SKILLS AND ABILITIES**:

- Working knowledge of human service and public health programming and services.
- Working knowledge of project management theories and techniques.
- Ability to gather data and analyze health indicators and risks to prioritize the most needed health education programs.
- Ability to coordinate various functional groups in order to produce a program or service.
- Working knowledge of the state community health needs assessment program.
