FT, (40 hrs) position to coordinate the Danbury Hospital GIP admissions and referrals to the Center and Community, working collaboratively with referring departments at the hospital and Regional to insure a smooth transfer of care for the patient/family. Specific duties will include obtaining necessary paperwork from patient/family; answering questions from patients/family members, and referring departments at DH regarding Regional Hospice; confirming admissions that can be scheduled with Regional clinical staff (i.e., RN-Case Manager, Social Worker, etc.)
Must have **strong** customer service experience in a healthcare environment (i.e., hospital admissions department) and excellent communication skills. The ability to navigate fast-paced, challenging situations comfortably is required, and at least 3 years working in healthcare preferably hospice is also required.
