**Noble Hospice **is currently seeking a experienced LPN to be our **Intake Coordinator **to oversee our intake process for our hospice agencies in Phoenix. The Intake Coordinator will be based out of either our West valley office in Glendale or our East valley office in Mesa, dependent on the location of the applicant.

**Responsibilities**:

- Receives new patient referrals and uses data received from referral to determine the best course of action for patient
- Coordinates referrals from hospitals, physicians, other health facilities, community agencies and patients and their families between patient account center and the clinical nursing team.
- Collaborate and develop workflows and processes to meet new regulatory requirements
- Monitor progress and delegate employee assignments
- Ensures processing of paperwork for new patients if efficient and in accordance with protocols
- Collaborates with multi-disciplinary team to facilitate and coordinate admission plans for patients coming into service
- Communicates with referral sources to ensure smooth transition of services and communication with patients
- Ensures that all admission forms are properly signed and filed
- Communicates with other team members constructively to reduce conflict and enhance the resolution of issues
- Respects clients or patient’s dignity and confidentiality
- Work closely with both clinical and business development teams
- Demonstrates advanced knowledge of home health and Hospice eligibility criteria and COP for purposes of appropriate referral coordination.
- Assists with admission process as needed within scope of license.

Qualifications:

- **LPN or RN License Required**:

- **Previous intake experience required**:

- **Hospice experience highly preferred**:

- Strong attention to detail and data entry experience required
- Experience in data management or a similar field preferred
- Understanding of different payor sources and ability to reach physician orders highly required
- Proficiency in Microsoft Office Applications including Word, PowerPoint, and Excel and other relevant software
- Experience with ALLSCRIPTS, NAVIHEALTH and other referral sources preferred
- Experience with insurance verification and EMR a plus

Pay: $82,000.00 - $85,000.00 per year
