**Responsibilities**:
Our House Calls program is growing in the Chicago suburbs Come join us in bringing outpatient primary care to patients in their homes.
- Cover assigned territory to care for patients in their homes
- Chronic care management
- Yearly assessments
- Weekday clinic schedule
- Work on a team with Physicians and NP's
- Mileage reimbursement
- Excellent benefits, PTO and retirement plans
- Malpractice insurance with tail coverage included

Qualifications:
**REQUIRED QUALIFICATIONS**:
**Education/Training**- Graduate of an accredited nurse practitioner or physician assistant program-
- Masters degree in Nursing or Physician Assistant Studies-
- Current Illinois nurse practitioner or physician assistant license in good standing-
- Certified as a nurse practitioner or physician assistant by an approved certifying organization.-
- Prescriptive authority, Illinois Controlled Substance, DEA certification-
- Current CPR certificationCome to be part of the world-renowned University of Chicago Medicine system, while still working in community medicine
