**Qualifications**
Master’s Degree in Social Work (MSW)
**Job description**
- Responsible for and participates in service delivery components of the Home Sharing & Respite Care Programs, follow-up, outreach, entitlements and benefits assistance, advocacy, etc.
- Prepares all required reports on a timely basis
- Conducts home visits, Host/Guest/HHA interviews, scheduling, evaluations, case management, crisis intervention and follow-up services for clients
- Assessment and matching of clients with appropriate home care workers and extensive follow-up
- Establishes and maintains working relationships with Program Director as well as appropriate City and State agencies and community organizations
- Insures implementation of procedures and policies
- Handles intakes for new clients for the Home Sharing & Respite Care Programs
- Prepares and organizes case files
- Verifies all references for Home Sharing & Respite Care applicants and records verifications in case file
- Refers clients as necessary to appropriate program related governmental entitlements and/or social service programs
- Other tasks as assigned
