**About the Role**

In keeping with our organization’s goals, the Director of Admissions will manage the nonclinical components of the referral process for multiple buildings.

**Job Duties**
- Enters new and return leads into CRM in PointClickCare.
- Verifies benefits through PCC for Medicare/Medicaid and through GCHMO’s ClickUp Portal for Managed Care.
- Runs NSOR report on all referrals.
- Ensures communication with Resident Services Coordinators to maintain awareness of occupancy levels and bed availability necessary for MOT.
- Understands the referral process including policy, procedures, assessments, and communication requirements.
- Participates in daily CRM calls.
- Updates lead status in CRM throughout the referral process.
- Inform RDCTs of referral source issues that arise during the referral process.
- Coordinates MOT notification with facility on all incoming admissions
- Maintain a thorough knowledge of the facilities services, clinical capabilities, and physician relationships

**Required Education, Experience, and Skills**
- 2 years’ experience with admissions process at a skilled nursing facility, subacute, rehab, or other health care facility.
- Significant experience and strong skills using Point Click Care software.
- Significant experience and strong computer literacy skills (Microsoft Outlook, Microsoft Office suite, and other online platforms).
- Recent long-term care experience required.
- Experience making independent decisions when circumstances warrant such action.
- Self-motivated and self-directed: possesses the ability to work independently
- Excellent written and verbal communication skills

Pay: $65,000.00 - $75,000.00 per year
