**Responsibilities**:

- Patient contacting
- Chart creations
- Lead contacting
- Patient prep for procedures
- Patient Support: Patient care coordinators serve as a point of contact for patients and their families, offering support and guidance throughout their healthcare experience. They may assist with appointment scheduling, answering inquiries, and providing information about medical procedures or treatment plans.
- Care Coordination: Coordinating patient care involves working closely with various healthcare professionals, such as doctors, nurses, therapists, and social workers. Patient care coordinators ensure seamless communication and collaboration among the care team members to develop and implement comprehensive care plans tailored to the individual patient's needs.
